What is Disability?
"Disability" or "disabled" means that
because of injury or sickness you cannot perform each of the material duties of your
regular occupation or, while unable to perform all of the material duties of your regular
occupation on a full-time basis, you are performing at least one of the material duties of
your regular occupation or another occupation on a part-time or full-time basis and are
earning currently at least 20% per month less than your indexed pre-disability earnings
due to that same sickness or injury.
After 24 months of payments, you are disabled when Unum
determines that due to the same sickness or injury, you are unable to to perform the
duties of any gainful occupation for which you are reasonably fitted by education,
training and experience.
Coordination of Benefits
Benefits payable under this plan are coordinated with
benefits payable under other disability coverages and retirement plans. Coordination of
benefits means that when other insurance plans or social programs (such as Workman's
Compensation, Social Security, or other group insurance plans) pay a benefit on the same
disability, the LTD plan coverage may be reduced or cease.
Pre-Existing Conditions
A "pre-existing condition" means a sickness or
injury for which the insured received medical treatment, consultation, care or services
including diagnostic measures, or had taken prescribed drugs or medicines in the three
months prior to the insured's effective date. The policy will not cover any disability
which begins in the first 12 months after an insured's effective date, if that disability
occurs as a result of a pre-existing condition.
Effective Date If application is made during your eligibility period,
acceptance in the plan usually begins the first day of the following month.

Plan Benefits

66 2/3% of Pre-Disability Income -
paid to you each month. The maximum monthly benefit is $6,000 and the minimum is $100.

9% of Pre-Disability Income to Retirement Plan -
put into your PCA Retirement Plan account each month for those
enrolled and contributing to the account prior to disability.

75 2/3% Total Annual Benefit

Cost-of-Living Adjustment - As the
cost of living goes up, so does your monthly benefit (based on an annual increase of up to
three percent 3%).

Survivor Benefit - Your survivor
will receive a benefit equal to three times your monthly benefit if you were disabled for
180 days or more and were receiving a benefit.

Rehabilitation -
Unum will normally cover rehabilitation costs should you become disabled.

90-Day Elimination Period - Benefit
payments begin ninety days from the date of disability.

Zero-Day Residual Benefit - allows
you to satisfy the elimination period with either a partial or total disability.

Own Occupation to any Occupation - If
you are disabled and cannot function in your current job, your benefits will continue for
24 months. Thereafter, in order to continue receiving benefits, you must be unable
to perform the tasks of any occupation for which you are qualified by training or
experience. If unable to perform the functions of any such occupation, benefits will
continue until age 65 or longer if disability begins after age 60.

Conversion Privilege - If you leave
the employment of the denomination or, in the case of an ordained pastor, are without
call, this group insurance plan is convertible to an individual policy.

Housing Allowance - For the
ordained minister, benefits received are available for housing allowance exclusion—up
to 100%. Disabled ministers are responsible for substantiating housing expenses on tax
returns.
